The Wisconsin Council on Physical Disabilities (CPD) partnered with the Governor's Committee for People with Disabilities (GCPD) to develop this toolkit of resources for health care providers and people with disabilities to address healthcare accessibility during an emergency. This toolkit provides a compilation of downloadable resources including checklists, planning forms, and communication tools in the case of an emergency. This toolkit is availble in English and Spanish and Braille (limited number of copies).
