Description
The Nevada COVID-19 Aging Network Rapid Response Plan provides ideas and resources for state and local aging service providers. To address social isolation, the plan includes a Social Support Action Team (SSAT) that provides social support to older adults by increasing their engagement in meaningful social interactions and assessing other social and community needs. SSAT provides one-on-one check-in calls, and small-group peer-support opportunities. This service will include a medical team, a social support team, a telehealth option, as well as a website.
